PHILHEALTH



Most employers nowadays are becoming cautious and specific with certain Pre-Employment Requirements
 
One of which is getting this accredited form submitted to assure your current employer of your PHILHEALTH number and other pertinent information:
 
  • Date of Birth
  • Place of Birth
  • Civil Status
  • Tax Identification Number (TIN)
  • Member Category
  • Sub-category
  • Previous employer information
  • Dependent(/s) Information table
Note that your PhilHealth remittances are STATUTORY at a certain extent (mandatory and hence, mandated by law) therefore getting this sheet may require you to at least go to their office at the earliest time and finish early. 

PHILHEALTH remittances not at all statutory

At a certain extent would include most private companies and government offices. If there are jobs which do not give out PHILHEALTH, PAGIBIG and SSS remittances, these would be project-based employees and low-income earning jobs by which the government is providing these employees with other extension programs which can serve as a better alternative. 
This post focuses on region VII (Cebu) and the PhilHealth office is situated near Golden Peak in Escario Street junctioning with Gorordo (ground floor).

If you wish to claim, please go upstairs and surrender to the guard on-duty any valid ID you have. If you don't have any idea how a valid ID looks like, click this link over here.

The offices upstairs (4th floor and 8th floor) extends it help in processing claims by members and other specific tasks like MDR requests, too. 
 
Now, here are some quick steps to begin with. 
 
1. Go to the nearest PhilHealth office and get your PRIORITY number. This is in GOLDEN PEAK HOTEL at the tail-gate of Escario Street, Cebu City. 
Whether you came in to check your MDR, or pay your bills (check other posts for this section), you have to line up here so that someone can assist you. 
 
2. Wait for your number to be called. Check the flashing numbers on the screen to know if it's already your turn. There are MORE counters serving hence it will just be really quick to wait. The office is fully airconditioned with restrooms near the waiting area.  
 

  • Priority lane for PWDs and pregnant women 
  • Another lane for just requests on MDRs
  • Another lane for payment, etc. 
This photo below shows those waiting in line for their turns. The system is over-all systematic and highly commendable. 


3. When your number is called, go to the counter as specified on the screen. There's a checklist before I forget which you will have to fill up pertaining to your purpose. 
4. In my case, I requested for an MDR and my first ever PHILHEALTH ID. 
5. In just half an hour, I was amazed by the process. 
No more longer hours for waiting.
